Dish Installation Checklist
1. Activation Approval - Read and complete with customer prior to installation the AFE
Installation Form. (You can obtain an AFE Installation Form for Dish Networks
Claim Form by visiting our website @ www.AFEinc.com.
Please print the form
and fill it out accurately.)
2. Call & Fax AFE’s Installation Form to AFE (615-859-9490) for credit approval.
Then wait for AFE to fax back the approved Customer Agreement Claim Form.
3. After Receiving approval, Installer will contact customer within 24 to 48 hours to set
up installation.
4. Check Equipment - Need pole and concrete for pole mount. We also offer tripod
installation for apartment balconies.
5. Call for Customer activation and programming choice’s and payment procedures after
installation.
6. Check and make sure that the TV and VCR are connected and working properly.
7. Explain the remote control functions with customer and Pay-per-view
activations.
8. Get customer’s signature on the Check off list and Dish Network Customer Agreement
Claim Form, including the R00 #‘s and S00 #‘s, then send originally signed forms
