Dish Installation Checklist
1. Activation Approval - Read and complete with customer prior to installation the AFE
Installation Form. (You can obtain an AFE Installation Form for
DISH Network's
Claim Form by visiting our website
at
www.afeinc.com.
Please print the form
and fill it out accurately.)
2. Call &
fax the Installation Form to AFE (615-859-9490) for credit approval.
Wait for AFE to fax back the approved Customer Agreement Claim Form.
3. After
receiving approval, installer will contact customer within 24 to 48 hours to set
up installation.
4. Check
equipment i.e., pole and concrete for pole mount, tripod
installations for
apartment balconies, etc.
5. Call for
customer activation and programming choices and payment procedures
after
installation. Customer will be asked to speak to DISH Rep for
verification.
6. Check
to make sure that the TV and VCR are connected and working properly.
7. Explain the remote control functions with customer and Pay-per-view
options.
8. Get customer’s signature on the Check-off list and DISH Network Customer Agreement Claim Form, including the R00 #‘s and S00 #‘s, then send
original signed forms to:
