                  MyToyBulldog's  Puppy Application:

                  Thank you for your interest in MyToyBulldogs. Although the number of litters produced
                  will be extremely limited, the quality of the breedings will be exceptionally high, using only AKC Champion
                  bloodlines with proper health clearances as stated in the BCA Guidelines.

                  It is our goal to produce dogs that will add to and improve the over all quality of the breed.
                  If you would like to be on file as a possible "MyToyBulldog Puppy Owner", please take a few minutes to   
                  complete this application. Snail mail it back to us:

                  This does not guarentee you a pup, but it does let us know you are interested. Thank you for your interest.

                  Please fill out the questions below.

                  1) What is it that attracts you to the Bulldog?
                  2) How did you learn of the breed?
                  3) What is your experience with this breed of dogs?
                  4) Please provide me with the names, breed, description of all pets you have had within the past 10 years.
                  Are they still with you. If no, why not?
                  5) Have you ever given up ownership of a pet to Animal Control, SPCA, or re-homed a dog? Why?
                  6) Please provide me with the name, address and phone number of your current vet. If you currently do not
                  have one,please provide me with the same information of a previous vet. Please know that I will check to
                  confirm this information.
                  7) Are you a current member of the Bulldog Dog Club of America? If not, will you join? (visit
                  www.bca.com)
                  8) Please tell me your expectations of me, should you receive a MytoyBulldog's puppy?
                  9) What activities do you plan on participating in with your dog?
                  10) What is your home arrangement? All MyToyBulldog puppies must have a home that has a fenced in yard
                  throughout their entire lives. "Invisible Fencing", or things similar to this, are not acceptable.
                  11) Will you agree on "puppy progress reports", keeping me up to date on the puppy's health, wellness,
                  adjustments, and successes as well as struggles? Will you let me know any change of addresses and
                  phone with 14 days of the changes?
                  12) All MyToyBulldog puppies will come with a contract.
                  13) At anytime, in the dogs life, that you can no longer provide a stable home environment, the dog must be
                  returnedto me. Will you agree to this?
                  14) Throughout the life of a Bulldog, nutrition and supplements are vital to healthy growth and over all
                  wellness. Willyou agree on a nutrition and food regiment, designed specifically for your MyToyBulldog   
                  puppy?
                  15) Socialization and training is a must for these bulldog breeds. Will you agree to enroll your
                  puppy in "Puppy Kindergarten" and at the least, a follow up with the completion of sub-novice obedience
                  before the dog is 9 months old?
                  18) I will not to ship puppies via airlines. Are you able to come and visit the puppies and then travel here to
                  pickthem up at the age of 8 to 10 weeks old?
