Tips for your Registration Transfer
Do not cross or scribble anything out. It will red flag your transfer and could result in filling out additional forms or having to obtain a duplicate .
Make sure your signature and mine are in place and dated.
Make sure all necessary questions are filled out including Date, Breed, sex, color, name, owner name & address and are printed clearly.
When naming your poodle, originality is a must. No one else in the entire USA can have (or ever has had) the same name. A plus is to begin it with something that would make it distinct from most, such as initials or something cutesy that is representing of you. (Examples: "Rebecca’s __________" or "RW’S ___________") This would greatly narrow down any national duplication.
Don’t lose or accidentally destroy it. Duplicates, no matter the reason, cost $15 each.
