Print these on either regular letter-sized paper or better yet, cardstock (if you have it). The stiffer paper means the tags won't bend and crease as much. You can also print these on the self-adhesive label paper if you prefer, for a different idea. There are many possibilities.
Then after printing your tags, simply write in your name and if you prefer, your e-mail address and attach to the item you wish to give. These can be attached to any type of item you make and give away as a gift. Items can be crocheted, knitted, tatted, sewed, hardangared, quilted, embroidered, made in the kitchen like gifts in a jar, cookies, breads, candies, etc. These tags will add a unique touch to all your gift-giving.
