Award Entry Form For You
Some people prefer not to use Free Form Providers for their Award Entry Forms, but do not know how to create their own Award Entry Form. To help you out on this if you are one of the ones that do not know how to create an Award Entry Form, then I have provided for you the code where you can copy the code and paste it into your HTML and your Award Entry Form will be at your Website.
Please don't use the below form to apply for Mesweet's Awards, because I will not accept it, if you do. The below Award Entry Form is only a sample to show you what the Award Entry Form will look like if you paste the below code into your HTML. You must put in your own email address if you want it to work for you. Please Note: The Award Entry Form will be the same color as your background that you have on your Website, unless you add a background color to the table.

Copy and then paste the below code into your HTML to create your own Award Entry Form. Be sure to put your email address where it says you@yourplace.com. Do not take away the question mark if you want your subject to be Award Application. The form is configurable. You can change the table, the border sizes and colors. You can add radio and text areas. If you prefer you can use it just as it is and it will look like the above Form, but remember you must put in your email address before pasting to your HTML, if you want it to work. It's highlighted in red to make it easy for you to locate.
